The Applicant’s parents or legal guardians should complete PART 2 of the application with neatness and accuracy, sign PART 2, and either include it with the application submitted by the Applicant or return it, together with the required attachments, directly to the Selection Committee at the above address no later than April 1. Additional pages may be attached as necessary if the space provided is inadequate. Information supplied will be kept confidential.
The Applicant should request two individuals to each complete a copy of PART 3. These individuals may be teachers, coaches, activity leaders, or other adults who know the Applicant personally but who are not related to the Applicant. The individuals should either complete the recommendation form in PART 3 or write a separate recommendation letter, sign the recommendation, and either return it to the Applicant to be included with the application or return it directly to the Selection Committee at the above address no later than April 1. Recommendation letters are not confidential.
Questions regarding the application process should be directed to Selection Committee. Recipients will be notified by mail on or about the date of graduation ceremonies.
